Over the past 60 years in the US, the incidence of diabetes increased more than sevenfold, making it one of the country’s most troubling health threats. Diabetes can cause heart disease, kidney failure and blindness, costing the US healthcare system and employers $237 billion annually.

Core Health Partners’ Diabetes Self-Management (DSME) program is designed to support and educate those living with Type 1 or Type 2 diabetes. With a medical referral, DSME is covered by Medicare, Medicaid and most private insurance providers.
